Death Records
The Smith County Clerk’s office has records for deaths that occurred outside the city limits of Tyler in Smith County only.

The Northeast Texas Public Health District has records for deaths that occurred inside the city limits of Tyler only. Their phone number is 903-535-0030 or 903-535-0031.

If the person died in another county in Texas, contact the County Clerk’s office in the county where the death occurred, or the State Bureau of Vital Statistics in Austin at (888) 963-7111.

Death records are closed to the public for 25 years. Certified copies can only be obtained by an immediate family member or his/her legal representative. You must present your valid state or government issued photo ID.

The fee is $21 (cash, money order, or cashier’s check) for the first certified death certificate and $4 for each additional certificate requested at the same time.
